4. Client Responsibilities

Maintain Confidentiality: Respect the privacy and confidentiality of any information shared by other individuals if participating in group sessions or if such information is inadvertently encountered.
Appropriate Conduct: Engage in respectful and appropriate conduct during all interactions with our practitioners and staff, whether online or in-person. Abusive, harassing, or disruptive behavior will not be tolerated and may result in termination of services.
Technology Requirements for Telehealth: Ensure you have the necessary equipment (computer, tablet, smartphone), a stable internet connection, and a private, quiet environment for telehealth sessions. We are not responsible for technical issues arising from your equipment or internet service.
Emergency Contact & Location: For telehealth services, you agree to provide your current physical location and an emergency contact person's name and phone number at the beginning of each session. This is crucial for safety protocols in case of an emergency.
Financial Responsibilities: Adhere to the payment policies outlined by FlexAbility Therapy Solutions, including timely payment of fees, co-pays, and deductibles as applicable.
Adherence to Treatment Plan: Cooperate with your occupational therapist and adhere to the agreed-upon treatment plan to the best of your ability.
5. Telehealth Specifics
By engaging in telehealth services with FlexAbility Therapy Solutions, you acknowledge and agree to the following:
Informed Consent: You will be provided with a separate Telehealth Informed Consent Form that outlines the specific risks, benefits, and limitations of telehealth, including emergency procedures. Your explicit consent will be required before commencing telehealth services.
Technology Limitations: You understand that telehealth relies on electronic communication and technology, which may have inherent limitations such as technical glitches, internet connectivity issues, or interruptions that could impact the quality or continuity of care.
Privacy and Security: While we use HIPAA-compliant platforms and implement robust security measures, no electronic communication is 100% secure. We will take all reasonable steps to protect your privacy as detailed in our Privacy Policy.
Emergency Procedures: You understand and agree to the emergency protocols discussed in your Telehealth Informed Consent Form, including providing your location and emergency contact information at each session.
Clinical Appropriateness: Your therapist will determine if telehealth is clinically appropriate for your specific needs. If it is deemed not appropriate, or if your condition requires in-person care, you may be referred to alternative services.
6. Payment and Billing
Fees: You agree to pay all fees for services rendered by FlexAbility Therapy Solutions as agreed upon during your intake process or as communicated prior to service delivery. Our fee schedule and accepted payment methods will be provided to you.
Insurance: We will assist with insurance claims as per our billing practices. However, you are ultimately responsible for understanding your insurance benefits, including co-pays, deductibles, and any non-covered services. You are responsible for any charges not covered by your insurance.
Cancellation Policy: You agree to adhere to our cancellation and no-show policy, which will be provided to you separately. Failure to provide adequate notice for cancellations or missed appointments may result in a fee.
